This panel conversation will feature ACH speakers from the Ventura County Community Health Improvement Collaborative and Sonoma County’s Health Action Together, alongside Managed Care Plan (MCPs) and Local Health Jurisdiction (LHJ) leaders.
Ventura County Community Health Improvement Collaborative
The Ventura County Community Health Improvement Collaborative is a formal, charter-bound collective impact partnership led by a Steering Committee of ten key health system stakeholders in Ventura County. These local hospitals, county health departments, community clinics, and Medi-Cal Managed Care plans first came together in 2018 to develop a collective Community Health Needs Assessment (CHNA) and strategic plan for health improvement across Ventura County. The full Collaborative now includes hundreds of stakeholders including community based organizations and residents. VCCHIC operational coordination, strategic planning, partner outreach, and evaluation are guided by its backbone organization – Community Lifting Communities.
Health Action Together & Sonoma Connect|Sonoma Unidos
Health Action Together & Sonoma Connect|Sonoma Unidos is a collaborative of community leaders dedicated to ending health and social inequities in Sonoma County. Health Action Together & Sonoma Connect is committed to co-creating a healing-centered ecosystem with communities facing systemic harm, by uplifting and centering community leadership and priorities, building a culturally responsive infrastructure, and cultivating cross-sector collaboration. People most impacted by racism are driving our efforts, including with the implementation of the Agenda for Action and the development of a local Community Information Exchange Model Network.
