This session will explore how Accountable Communities for Health (ACHs) serve as a bridge between communities, managed care plans, and local health jurisdictions—and why community leadership is essential to shaping Community Health Needs Assessments (CHNAs) and Community Health Improvement Plans (CHIPs). We’ll share examples and practical strategies to ensure residents are at the table throughout assessment, planning, and implementation, as well as in public health investment discussions.
