First released in 2019 and refreshed with 2025 insights, it provides practical guidance for designing, governing, and sustaining locally controlled wellness funds as part of the Accountable Communities for Health (ACH) model, a multisector, collaborative approach to advancing community health and equity.

Highlights include:

  • Why wellness funds matter for equity and sustainability

  • Real-world examples from San Pedro & East San Jose

  • Models for blending, braiding, and aligning funds

  • 10 key steps for building your fund

  • Recommendations for policymakers & funders

Explore the brief and tell us what you think. This resource is especially valuable for multisector collaborative leaders, funders, and policymakers who are working to embed equity and sustainability in their communities—and, especially now, for those seeking creative ways to invest in collaboration and pool resources, recognizing that funding equity work together is often more effective than going it alone.
