Building Stronger Connections Across Our Community

The Humboldt Community Health Trust (HCHT) has served as Humboldt County's Accountable Community for Health (ACH) since 2016. Established through the California Accountable Communities for Health Initiative (CACHI), HCHT brings together organizations from across sectors to solve problems that no single agency can address alone. What started as a collaborative focused on substance use disorder has evolved into a broader effort to strengthen how organizations work together to support the health and well-being of our community. Today, HCHT serves as a trusted convener, bringing together healthcare providers, community-based organizations, tribal partners, schools, local government, philanthropy, and other community leaders to build relationships, align priorities, and take collective action.

Relationships First

The strength of HCHT has always been its ability to create space for collaboration. Partners come together not because they are required to, but because they recognize that lasting solutions depend on shared ownership and trusted relationships. HCHT provides a neutral table where organizations can have honest conversations, identify common goals, learn from one another, and work through challenges together. Over time, those relationships have become the foundation for new partnerships, joint funding opportunities, and regional initiatives that would not have happened otherwise.

Turning Collaboration into Shared Infrastructure

As trust has grown, so has the community's ability to build shared solutions. One of HCHT's most significant accomplishments has been supporting the development of North Coast Care Connect, the region's Community Information Exchange (CIE). The CIE allows organizations from many different sectors to coordinate services, make electronic referrals, and share information in ways that help people navigate systems more easily. Instead of asking individuals to tell their story over and over, partners can work together around shared goals and provide more coordinated support. HCHT is also leading the development of a regional Community Care Hub (NCHIIN Shared Services) that strengthens the capacity of community-based organizations to participate more fully in coordinated systems of care. Through shared administrative services, technical assistance, and opportunities for contracting support, NCHIIN’s Shared Services helps organizations focus more of their time on serving people while creating a stronger network of support across the region. Together, the Community Information Exchange and Community Care Hub represent more than new programs. They are community-owned infrastructure that makes collaboration easier, strengthens relationships between organizations, and creates lasting capacity for the region.

HCHT continues to bring partners together around the issues that matter most to our community. Whether convening leaders, supporting regional planning, strengthening community-based organizations, or investing in shared infrastructure, HCHT remains focused on one simple idea: when organizations work together, communities are better able to respond to complex challenges and create lasting change.
