10 Years of Impact: 2016-2026
Over the past decade, CACHI has helped 36 Accountable Communities for Health take root across 27 California counties—engaging more than 1,000 cross-sector organizations and aligning over $30 million in public and private funding for community health and prevention. What began as a pilot to test a new model of collaboration has become proof of what communities can achieve when they have the infrastructure to act together.
ACHs have responded to crises, changed policies, coordinated care across sectors, and placed residents at the center of decisions that shape their health. While each ACH reflects the unique priorities and strategies of its community, they are united by shared principles of equity and community voice, learn from one another as a peer network, and are designed as lasting systems built by and for the communities they serve.
