Health System Brief

This brief shows how California’s Accountable Communities for Health (ACHs) are helping health systems work more effectively with communities under CalAIM and through the CHA/CHIP process. It explains how ACHs act as trusted backbone infrastructure—convening managed care plans, public health, and community organizations; building Community Health Worker capacity; improving data sharing; and centering community priorities. Drawing on examples from across the state, the brief illustrates how ACH partnerships can drive more coordinated, equitable, and sustainable health system transformation.
